Guardian Consent

保護者同意書（未成年のお子様のご参加について）

When a minor (under 18) joins a tour, we need a parent or guardian's consent. This page sets out what you confirm and agree to on the child's behalf. Agreement is captured by a consent checkbox at booking, or by signing this form in person.

未成年（原則18歳未満）のお子様がツアーに参加される場合、保護者の方の同意が必要です。本ページは、お子様に代わってご確認・ご同意いただく内容を定めるものです。同意は、ご予約時の同意チェック、または本書面へのご署名により取得します。

1. Participant & Guardian

  • Child's name and age (for each child joining)
  • Guardian's name, and relationship to the child
  • Contact (phone, email)
  • Emergency contact — someone not joining the tour
  • Whether the guardian will accompany the tour, or entrust the child to the guide
  • お子様の氏名・年齢（参加されるお子様全員）
  • 保護者の氏名・お子様との続柄
  • 連絡先（電話・メール）
  • 緊急連絡先（ツアーに同行されない方）
  • 保護者の同行の有無（同行する／同行せず引率を依頼する）

These details are collected through the booking form.

2. Acknowledgement & Consent

As the guardian, I consent to all of the following:

  • I am the legal guardian of the child named, I consent to their participation, and I accept responsibility as their guardian.
  • I have accurately declared the child's health, fitness, allergies, and any medical conditions. The child is in a suitable condition to take part.
  • I understand that outdoor activity carries inherent risks (trips, falls, wildlife, sudden weather changes) and accept these on the child's behalf.
  • I agree, on the child's behalf, to OUTSCAPE's Terms & Conditions and Waiver.
  • I agree to the Cancellation Policy.
  • I consent to emergency first aid, and transport to a medical facility, if the guide judges it necessary.
  • I consent to the handling of the child's personal information (including special care-required information) under the Privacy Policy.
  • I consent to promotional use of photos and video of the child (opt-out available — just tell the guide).
  • I will ensure the child follows the guide's instructions. This consent remains valid whether or not I accompany the tour.
  • 本人は上記のお子様の法定保護者であり、お子様のツアー参加に同意し、保護者としての責任を負います。
  • お子様の健康状態・体力・アレルギー・持病等を正確に申告しました。お子様はツアーに参加できる健康状態にあります。
  • 屋外アクティビティには固有のリスク（転倒・転落・野生動物・天候の急変等）があることを理解し、お子様に代わって承諾します。
  • OUTSCAPEの利用規約・免責事項（Waiver）に、お子様に代わって同意します。
  • キャンセルポリシーに同意します。
  • 緊急時、ガイドが必要と判断した応急処置および医療機関への搬送に同意します。
  • プライバシーポリシーに基づくお子様の個人情報（要配慮個人情報を含む）の取り扱いに同意します。
  • お子様がツアー中に撮影された写真・動画の広報利用に同意します（ご希望されない場合はガイドにお伝えください）。
  • お子様がガイドの指示に従うよう監督します。保護者が同行しない場合も、本同意は有効です。

3. Signature

For online bookings, your consent checkbox at booking takes the place of a signature. The block below is for in-person use.
